6/1/2023 EnchantaVaughn Approved (AM) Client responded well to review and was confident he can meet ISP objectives. Client communicated positive affirmations and self talk. Client stated he is tired in the morning because he goes to bed late. Client understands it is his responsibility to prepare himself for school each day. Client acknowledges that he has to be more respectful to his teachers and peers before expecting to be treated fairly. Client modeled appropriate behavior in class by focusing on his work and remaining in his seat. Client needed redirection to get back on track with goals and objectives. (PM) Client asked appropriate questions and talked about his challenges and hardships at school. Client explains that his peers are aggressive and mean and always pick on him. Client was supported with listening and following directions despite stressful environment. Client processed his feelings with QMHP. Client engaged in self reflection. Client reflected about his experiences in school. Client practices mindfulness with 5 senses (i.e. listening, seeing, smelling, tasting, and movement). Client is observed being courteous and polite and showing excellent manners by saying please and thank you. Client attempts to build new peer relationships. Client is seen being a leader to his peers when motivated by a reward. Client appears to be more self aware and confident with verbalizing his thoughts, and the benefits of this were reviewed. (EOD) Client smiled and said thank you. Client was motivated to be responsible and take accountability for his actions due to empowering conversation with mental health professional. Client was able to process his choices and correct his mistakes. Client was observed being apart of the solution instead of the problem. Client took a 30 minute break to organize daily activities and relax before returning back to academic setting. Client said okay and nodded in agreement. Client promises to be calm even if things don't go his way because he wants to prove he can be good and be a leader in school, at home, and in the community.
